  • How do I submit a request for medical records, billing statements and/or x-ray (radiologic) images?
    Please note that this does not apply to patients requesting their own records. You can send a subpoena or authorization via mail, email, fax or A1 Dashboard. We require separate requests for medical records, billing statements, and x-ray (radiologic) images. A $15 Witness Fee is required for each request, in California only. Please make checks payable to A1 Copy Service.   • Can patients request their own medical records, billing statements and/or x-ray (radiologic) images?
    Absolutely! You have two options. Option 1: Fill out our Patient Request Authorization form and send it back via mail, email or fax. The records will be sent out after payment is received. Patient Request Authorization form is also available in Spanish. Click here for the form in English Haga clic aquí para ver el formulario en Español Option 2: - Click the link below, which will take you to our A1 Copy Service Dashboard. - Once you're on the dashboard, you’ll need to create an account by entering your email address and choosing a secure password. - After setting up your account, you can submit your record request directly through the dashboard. Be sure to provide all the required details for faster processing. - You can use the dashboard to track the status of your request at any time. dashboard.a1copyservice.com
  • How do I check status on a request?
    Send an email to Status@A1CopyService.com. Be sure to include the patient's full name, date of birth, and reference number (if applicable). You can also check status from our dashboard if you submitted your request via A1 Copy Service Dashboard. dashboard.a1copyservice.com Please note: We do not give status over the phone.
  • What's your turnaround time on regular and expedited* requests?
    For a regular request; please allow 7-14 business days to process, retrieve documents and send you an invoice. For an expedited request; please allow 1 business day to process, retrieve documents and send you an invoice. After payment is processed, the requested documents will be sent within 2 business days for a regular request or within 1 business day for an expedited request. *There is a $30 fee for each expedited request.
  • How do I pay an invoice?
    You can pay an invoice by mailing a check payable to A1 Copy Service: 17412 Ventura Boulevard, #800 Encino, California 91316 You can also pay an invoice with a credit or debit card using the link sent via email. Please note that there is a 4% expedite payment fee* for this option. *Disregard the 4% expedite payment fee if you would like to pay by check. Check payments with this fee added is at the expense of the payor.
